Oakleigh school transition documents and NDIS review reports

Do you need any upcoming reports or reviews?

With the festive season fast approaching, please speak to your clinician if you require any reports for the January – April 2025 period.

During December and January our clinicians have some additional flexibility in their calendars and can prioritise reports that you may need for the upcoming term.

These could include:

  • School transition reports – If your child is transitioning to Prep, a new school or high school, our school transition reports are a good way for your child’s therapist to introduce themselves to your child’s new teacher and provide background to your child’s therapeutic goals. We also provide classroom based strategies to support your child in this transition to make the process as smooth as possible
  • NDIS review reports – If your NDIS review is planned between December 2024-April 2025 our therapist can now commence writing your child’s review reports. With the recent changes to NDIS, provision of review report timelines have become much smaller. Our therapists typically require 2 weeks to complete a report so having this completed in advance can make the process less stress free.
  • NDIS access reports – If you are currently in the process of applying for NDIS funding, you will be required to provide reports therapists outlining your child’s needs. Our team can provide a summary report outlining your child’s therapeutic history and required supports moving forward to assist in this process.
  • Goal review summaries – Our therapists will be reviewing your child goals and progress over the December/January period. If you would like a progress summary to keep for your records, provide to you paediatrician or child’s external team, please speak to our team so we can finalise this for you.
